Title of a good movie i saw a long time ago?

I saw this movie a long time ago and i think it would be called a chick flick. It has mandy moore in it. and she is a geeky girl at the beginning and she starts hanging around the jock for some reason. i think she becomes his tutor they like eachother. he blows her off in front of friends. she gets pissed. he feels bad. he finds out all her dreams. he names a star after her. she wants to be in two places at once so he makes her stand on the state line so that she is in two states at once. he gives her a fake erfly tattoo because she has always wanted a tattoo but her father would never allow it. they love eachother. she has a disease and the guy doesn't know throughout most of the movie. she dies at the end of the movie. i know there is a book of it too. does anyone know what the title of the movie is?
